પ્રાથમિક & માધ્યમિક શાળાઓ માટે UDISE+ ભરવા બાબત માર્ગદર્શન. 

  સમગ્ર દેશની તમામ સરકારી અને ખાનગી પ્રાથમિક, માધ્યમિક, ઉચ્ચ માધ્યમિક તથા અન્ય શૈક્ષણિક સંસ્થાઓની માહિતી એકત્રીકરણ માટે પ્રતિ વર્ષ સમગ્ર શિક્ષા કચેરી તરફથી શાળાઓને Unified District Information System for Education Plus ( UDISE + ) ફોરમેટ પુરા પાડવામાં આવે છે. શાળા તરફથી મળેલ સાચી અને સચોટ માહિતીને આધારે શિક્ષા મંત્રાલય – નવી દિલ્હી તરફથી વિવિધ શૈક્ષણિક યોજના / કાયદાને આકાર આપવામાં આવે છે. – પ્રતિ વર્ષ માટેનું ( UDISE + ) ફોરમેટ મોકલવામાં આવે છે. તેમાં શાળાની કક્ષાએથી શું - શું કરવાનું છે, તેની વિગતવાર જાણકારી આ સાથે આપવામાં આવેલ છે. માર્ગદર્શક બાબતોને ધ્યાને રાખી, ( UDISE + ) ફોરમેટમાં સુધારવા લાયક બાબતોનો સુધારો કરી તથા ખૂટતી માહિતી ઉમેરીને અત્રેની કચેરીને મળી જાય, તે અંગે યોગ્ય કાર્યવાહી કરવી.

UDISE + ભરવા માટેની માર્ગદર્શક બાબતો 

* આ સાથે આપવામાં આવેલ UDISE + દરેક શૈક્ષણિક વર્ષ માટે ભરવાનું થાય છે. 

* તેમાં કોઇ માહિતી ખોટી હોય, તો તેની ફરતે લાલ પેનથી રાઉન્ડ કરવો. 

* રાઉન્ડ કરેલ ખોટી માહિતીની બાજુમાં સાચી માહિતી ફકત લાલ પેનથી જ લખવી. 

* જો કોઇ માહિતી ઉમેરવાની થતી હોય, તો તે ફકત લીલી પેનથી જ લખવી. 

* સુધારવાની કે ઉમેરવાની માહિતી ફકત અંગ્રેજી ભાષામાં જ લખાય, તે ઇચ્છનીય છે. 

* ભરાયેલ UDISE + આચાર્યશ્રીનું નામ, હોદ્દો, સહી અને સિકકો એકુન કરીને પરત કરવું. 

* ફોરમેટ પરત મોકલતી વખતે શાળાનો જાવક સિકકો લગાવી, જાવક નંબર અવશ્ય આપવો. 

* શાળાએ પોતાની પાસે એક નકલ અવશ્ય રાખવી, જેથી જરૂરીયાતના સમયે માહિતી મેળવી શકાય. 

* ખાનગી શાળાએ પોતાની એન્ટ્રી જાતે કરવાની રહેશે. UDISE + ની ઝેરોક્ષ સી.આર.સી. – તરસાઇને આપવી. 

વિભાગઃ— ૧ શાળાનું વિવરણ સ્થાન, સંચાલન અને શૈક્ષણિક માધ્યમ સહિત ) 

• શાળાનો યુ – ડાયઝ કોડ, અંગ્રેજીમાં શાળાનું નામ, જિલ્લો, તાલુકો, વિસ્તાર, ગ્રામ પંચાયતનું નામ વગેરે વિગતો તપાસી લેવી. 

• ગ્રામીણ શાળાએ મ્યુનિસીપલ કોર્પોરેશન / નગરપાલિકામાં માહિતી ભરવાની નથી. તે ફકત શહેરી વિસ્તાર માટે છે. 

• વોર્ડની વિગત ભરવની નથી. આ ખાનું ખાલી છોડવું. વિભાગઃ —૧ ના મુદ્દા નંબર ૧.૫ થી ૧.૮ ( બી ) સુધી તપાસી લેવા. જો ખાલી હોય તો અંગ્રેજીમાં ભરવા. 

• મુદ્દા નંબર ૧.૧૦ માં મોબાઇલ નંબર, મેઇલ એડ્રેસ લખવા. જો શાળાની વેબસાઇટ કે બ્લોગ હોય, તો દર્શાવવું. 

• મુદ્દા નંબર ૧.૧૧ માં કોડ નંબર – ૧ લખવો. ( બી ) માં આચાર્યનું નામ અને ( સી ) માં આચાર્યનો મોબાઇલ નંબર. 

• મુદ્દા નંબર ૧.૧૩ થી ૧.૧૫ ( એ ) સુધી તપાસી લેવા. લાગુ પડતા કોડ ત્યાં બાજુમાં આપેલા છે. 

• ૧.૧૫ ( બી ) સરકારી શાળા / ગ્રાન્ટેડ શાળા / ખાનગી શાળાએ ભરવાનું નથી. તે બન્ને ખાના ખાલી છોડવા. 

• મુદ્દા નંબર ૧.૧૬ ( એ ) ( બી ) ( સી ) ની ત્રણેય કોલમ ભરવાની છે. તેમાં લાગુ પડતી સાચી માહિતી ભરવી. 

• મુદ્દા નંબર ૧.૧૭ માં મોટાભાગની શાળાઓએ એક – એક વર્ગ દર્શાવવાનો થશે. જો વધારે વર્ગો હોય, તો તે મુજબ માહિતી ભરવી. 

• મુદ્દા નંબર ૧.૧૯ ( એ ) માં શાળા સ્થાપના વર્ષ લખવું તથા ( બી ) માં ધોરણઃ– ૮ મંજૂર થયા વર્ષ લખવું. 

• મુદ્દા નંબર ૧.૨૨ થી ૧.૨૯ પૈકી કેટલીક કોલમો ખાલી હશે. દરેક કોલમ ભરવી ફરજિયાત નથી. તમામ કોલમ વાંચી, જાણી ભરવી જો શાળાને લાગું પડતી હોય, તો જ ભરવી.

• ૧.૩૦ માં જે ભાષા જે ધોરણમાં શીખવવામાં આવતી હોય, તે લખવું ઉદાહરણઃ– ૩ ગુજરાતી ધોરણઃ— ૧ થી ૮ દરેક ભાષા શીખતા બાળકોની સંખ્યા 2022 માટે ૩૧ માર્ચ ૨૦૨૧ ની સ્થિતિ પરથી ભરવાનો રહેશે.

• મુદ્દા નંબર ૧.૩૧ થી ૧.૩૩ ( ડી ) ની કોલમ તપાસી લેવી. ભૂલ હોય, તો સુધારી લેવી. 

• મુદ્દા નંબર ૧.૩૫ ( એ ) – ( બી ) તથા ૧.૩૬ ( એ ) – ( બી ) ની વિગતો શાળા દફતરેથી ખરાઇ કરીને લખવી. સી.આર.સી. સાથે ચર્ચા કરી લેવી. 

• કોલમ નંબર ૧.૩૭ ( એ ) માં ૫.૦૦ કલાક અને ( બી ) માં ૫.૧૫ કલાક દર્શાવવું. 

• મુદ્દા નંબર ૧.૩૮ માં પ્રાથમિક ( ધોઃ —૩ થી ૫ ) માં લેવાયેલ ગત વર્ષની એકમ કસોટીની સંખ્યા લખવી. ઉચ્ચ પ્રાથમિક ( ધોઃ —૬ થી ૮ ) માં લેવાયેલ ગત વર્ષની એકમ કસોટીની સંખ્યા લખવી. 

• મુદ્દા નંબર ૧.૩૮ ( એ ) માં વાલી સાથે પરીણામની ચર્ચા કરવામાં આવે ત્યાં કોડ – ૧ લખવો. ( બી ) માં કોડ – ૨ લખવો . 

• મુદ્દા નંબર ૧.૪૧ માં કોડ – ૨ લખી, તેની નીચે આપવામાં આવેલ કોષ્ટક ખાલી છોડવું. 

• મુદ્દા નંબર ૧.૪૨ અને ૧.૪૩ માં કોડ – ૨ લખવો. કોલમ ( એ ) થી કલોમ ( એફ ) ખાલી છોડવી. 

• મુદ્દા નંબર ૧.૪૪ ( ઉપચારાત્મક શિક્ષણ ) અને ૧.૪૫ ( શીખવાની અભિવૃધ્ધિ ) માં વા.લે.ગ. માં આવરી લીધેલ વિદ્યાર્થીઓની સંખ્યા લખવી. 

• કોલમ ૧.૪૬ માં ( એ ) સ્કૂલ ઇન્સપેકટરની મુલાકાત, ( બી ) સી.આર.સી. ની શાળા મુલાકાત, ( સી ) બી.આર.સી. અને તાલુકા પ્રાથમિક શિક્ષણાધિકારીની મુલાકાત, ( ડી ) જિલ્લા / રાજય કક્ષાના અધિકારીની મુલાકાતની સંખ્યા લખવી. 

•  મુદ્દા નંબર ૧.૪૭ ( સી ) માં ગત વર્ષની એસ.એમ.સી. ની બોલાવવામાં આવેલ બેઠકની સંખ્યા લખવી. ( ડી ) માં કોડ – ૧ લખવો. નીચેવર્ષના ખાનામાં ૨૦૨૧ લખવું. 

• મુદ્દા નંબર ૧.૪૮ ( એ ) થી ( સી ) માં કોડ – ૨ લખવો. 

• મુદ્દા નંબર ૧.૪૯ શાળામાં ઉપલબ્ધ વર્ગખંડની સંખ્યા અને શાળામાં ભણતા બાળકોની સંખ્યાને ધ્યાને રાખી ભરવું. તેના પરથી નીચે આપેલ કોષ્ટક ભરવાનું રહેશે. ( જો હા હોય, તો જ ) 

• મુદ્દા નંબર ૧.૫૦ માં કોડ – ૧ લખવો. 

• કોલમ નંબર ૧.૫૧ ફકત તાલુકા શાળા / પગાર કેન્દ્ર શાળાએ ભરવાની છે. 

• મુદ્દા નંબર ૧.૫૨ અને ૧.૫૩ નો તમામ શિક્ષકમિત્રોએ સાથે મળી અભ્યાસ કરી ભરવાની રહેશે. જો કોઇ મુદ્દા વિશે સમજણ ન પડે, તો સી.આર.સી. – તરસાઇનો ટેલીફોનિક સંપર્ક કરવો.

વિભાગઃ– ૨ ભૌતિક સવિધાઓ, સાધનો, કમ્પ્યુટર અને ડીઝીટલ ક્ષેત્રે પહેલ 

ભાગ ( A ) શાળામાં ભૌતિક સુવિધાઓ અને સાધનોઃ

> મુદ્દા નંબર ૨.૨ ના કોષ્ટકમાં શાળા પરીસરમાં ઉપલબ્ધ બિલ્ડીંગનું સંખ્યાત્મક વિવરણ કરવાનું છે. 

> મુદ્દા નંબર ૨.૩ ના ખાનમાં નીચે પૈકી લાગુ પડતો કોડ લખવો. ખાનું ખાલી છોડવાનું નથી. 

> મુદ્દા નંબર ૨.૪ ( એ ) માં શાળામાં ઉપલબ્ધ રૂમનો ઉપયોગ વર્ગીકૃત કરવાનો છે. મુદ્દા નંબર ૨.૪ ( એ ) માં શાળા ના વર્ગખંડની સ્થિતિ સ્પષ્ટ કરવાની રહેશે. મુદ્દા નંબર ૨.૪ ( સી ) માં વધારાના વર્ગખંડ હોય, તો સંખ્યા દર્શાવવી . 

> મુદ્દા નંબર ૨.૫ ( એ ) માં મુતરડી / સંડાસની વિગતો આપવામાં આવેલ છે. તપાસી જરૂરી સુધારો હોય, તો કરવો. મુદ્દા નંબર ૨.૫ ( બી ) માં પાણીની સુવિધા હોય, તો કોડ – ૧ લખવો. પાણીની સુવિધા ન હોય, તો કોડ – ૨ લખવો. કોલમ ( સી ) અને ( ડી ) તપાસી લેવી હિતાવહ છે. 

> મુદ્દા નંબર ૨.૬ ( એ ) માં ઉપલબ્ધતા કોલમમાં કોડ – ૧ આવે, તો જ કાર્યરત કોલમમાં લાગુ પડતો કોડ લખવો. કોલમ ( બી ) અને ( સી ) આર.ઓ. પ્લાન્ટ વિશેની છે. 

> મુદ્દા નંબર ૨.૭ વરસાદી પાણીના સંગ્રહ / ભોં ટાંકા અંગેની છે. લાગુ પડતી માહિતી આ કોલમમાં લખવી. 

> મુદ્દા નંબર ૨.૮ માં હાથ ધોવા માટેની સગવડની કોલમ આપેલ છે. ખાલી હોય, તો સાચી માહિતી ભરવી. 

> મુદ્દા નંબર ૨.૯ ( એ ) માં કોડ – ૧ હશે. મુદ્દા નંબર ૨.૯ ( બી ) માં કોડ – ૨ હશે / લખવો.

> મુદ્દા નંબર ૨.૧૦ માં ડેડસ્ટોક રજીસ્ટરના પુસ્તક વિભાગ પરથી ઉપલબ્ધ પુસ્તકોની સંખ્યા લખવી. કોઇ શાળા બુક બેંક ચલાવતી હોય, તો તેની સંખ્યા લખવી. મુદ્દા નંબર ૨.૧૦ ( બી ) અને ( સી ) ની વિગત તપાસી લેવી. – 

> મુદ્દા નંબર ૨.૧૧ શાળા વિસ્તરણ માટે પરીસરની બાજુમાં જમીન હોય, તો કોડ – ૧ અન્યથા કોડ – ૨ લખવો. 

> મુદ્દા નંબર ૨.૧૨ શાળાને રમતનું મેદાન હોય, તો કોડ – ૧ લખવો . મેદાન વિહીન શાળાએ કોડ – ૨ લખવો. 

> મુદ્દા નંબર ૨.૧૩ ( એ ) થી ( ડી ) શાળા આરોગ્ય તપાસણી વિશે માહિતી ભરવાની છે. જો શાળામાં આ વર્ષ દરમ્યાન થયેલ હોય તો જ લાગુ પડતી વિગતો ભરવી. જો કોલમ ૨.૧૩ ( એ ) માં કોડ —૧ લખેલ હોય, તો જ ( ઇ ) થી ( જી ) કોલમમાં કોડ – ૧ લખવો અન્યથા ખાલી છોડવું 

> મુદ્દા નંબર ૨.૧૭ કચરા પેટી અંગેની ( એ ) થી ( સી ) કોલમમાં કોડ – ૧ લખવો. 

> મુદ્દા નંબર ૨.૧૮ વિદ્યાર્થીઓ માટે શાળામાં ઉપલબ્ધ ફર્નિચર વિશે લાગુ પડતો કોડ લખવો .

> મુદ્દા નંબર ૨. ૧૯ શાળા સુવિધાની વિવિધ માહિતી આપવાની છે. જો હોય તો કોડ –૧, ના હોય તો કોડ – ર લખવો. 

> મુદ્દા નંબર ૨.૨૧ કયા સાધનો છે, તેની માહિતી આપવાની છે, જો હોય તો કોડ –૧, ના હોય તો કોડ – ર લખવો. 

ભાગ ( B ) કમ્પ્યુટરર્સ અને ડીઝીટલ ક્ષેત્રે પહેલઃ 

✦ મુદ્દા નંબર ૨.૨૨ માં ( એ ) થી ( ઓ ) સુધીની માહિતી ભરવા કોડ લખવાનો નથી, પરંતુ ઉપલબ્ધ સંખ્યા અને ચાલુ કોય તેવા સાધનોની સંખ્યા લખવાની છે. ( ડેડ સ્ટોક રજીસ્ટર પર નોંધાયેલ સાધનોને ધ્યાને રાખીને ) 

> મુદ્દા નંબર ૨.૨૩ ઇન્ટરનેટ સુવિધામાં મોટાભાગની શાળામાં કોડ -૧ આવશે. ( નેટવર્ક ન આવતું હોય, તે શાળા એ કોડ – ર લખવો ) ઇન્ટરનેટના પ્રકાર માટે ફોર્મમાં નીચે આપેલ કોડમાંથી યોગ્ય કોડ પસંદ કરી લખવો. 

> મુદ્દા નંબર ૨.૨૪ ના કોષ્ટકમાં જો સુવિધા હોય તો કોડ -૧ અને સુવિધા ન હોય તો કોડ –૨ લખવો.

> મુદ્દા નંબર ૨.૨૫ માં ડીઝીટલ લાઇબ્રેરીના પહેલા ખાનામાં કોડ – ર લખવો. તેની નીચેનું ખાનું ખાલી છોડવું. 

> મુદ્દા નંબર ૨.૨૭ માં ના ICT પહેલા ખાનામાં કોડ – ર લખવો. નીચે આપેલ ( I ) થી ( IV ) ખાલી છોડવા. 

વિભાગ : - ૩ શૈક્ષણિક અને બીન શૈક્ષણિક સ્ટાફની વિગતઃ 

+ મુદ્દા નંબર ૩.૧ ( એ ) થી ( જી ) સુધી શાળામાં બીન શૈક્ષણિક સ્ટાફ જો ફરજ બજાવતો હોય, તો દરેકની સંખ્યા લખવી. અન્યથા ખાલી છોડી દેવું. 

+ મુદ્દા નંબર ૩.૨ માં શાળામાં હાલની સ્થિતિએ ફરજ નિયુકત શિક્ષકોની સંખ્યા લખવાની છે. ( એ ) કોલમમાં પુરા પગારવાળા શિક્ષકોની સંખ્યા, ( બી ) કોલમમાં વિદ્યાસહાયક અને પ્રવાસી શિક્ષકોની સંખ્યા, નીચે સરવાળો કરી કોલમ ( સી ) માં કુલ પૈકી કોઇ જાતિ બદલાવેલ શિક્ષક હોય તો કોડ –૧ લખવો, ના હોય તો કોડ – ર લખવો. 

+ મુદ્દા નંબર ૩.૩ નો અભ્યાસ કરી જવો. તેમાં દર્શાવેલ કોડ આગળ શિક્ષકની વ્યકિતગત માહિતીમાં વાપરવા. 

+ શાળામાં આજની તારીખે કાર્યરત શિક્ષકોની સીટનો ભાગઃ– A છાપેલ આપેલો છે. તેમાં કોઇ ભૂલ હોય, તો લાલ પેનથી સુધારી બાજુમાં લખવું. આ ભાગની કેટલીક કોલમ ખાલી છે, જે ફકત લીલી પેનથી જ લખવી. શિક્ષકોની સીટનો ભાગઃ– B નો વ્યવસ્થિત અભ્યાસ કરી ભરવો. ( આ માહિતી જે તે શિક્ષક પાસે ભરાવવી હિતાવહ છે ) 

+ ફોરમેટમાં દર્શાવેલ શિક્ષકો પૈકી કોઇની બદલી થયેલ હોય, તો વિગત ઉપર લાલ પેનથી ટ્રાન્સફર લખવું. નિવૃત થયેલ હોય, તો લાલ પેનથી નિવૃત લખવું.

+ બદલીથી આવેલ કે નવી નિમણૂંક થયેલ શિક્ષકોની માહિતી સાથે આપેલ કોરી સીટ ભાગઃ– A અને ભાગઃ— B બન્ને ફકત લીલી પેનથી જ ભરવા. 

વિભાગ : – ૪ નામાંકન અને પનઃપ્રવેશ ( રીપીટર ) : 

√ મુદ્દા નંબર ૪ માં જાતિ બદલાવેલ બાળકો હોય તો કોડ –૧, આવા બાળકો ના હોય તો કોડ – ર લખવો. 

√ મુદ્દા નંબર ૪.૧.૧ માં પૂર્વ પ્રાથમિકની આંકડાકીય માહિતી લખવી. સરકારી પ્રામિક શાળાઓમાં નહીં આવે. 

√ મુદ્દા નંબર ૪.૧.૨ માં ધોરણઃ -૧ ની તાઃ- ૩૧/૦૩/૨૦૨૨ ની સ્થિતિએ સંખ્યા દર્શાવવી.

√ મુદ્દા નંબર ૪.૨ ( એ ) માં ધોરણઃ —૧ થી ધોરણઃ —૮ સુધીની જાતિવાર / કુમાર કન્યાવાર તાઃ- ૩૧/૦૩/૨૦૨૨ ની સ્થિતિએ સંખ્યા લખવી. આંગણવાડી ન હોય, તો તે ખાનું ખાલી છોડવું. 

√ મુદ્દા નંબર ૪.૨ ( બી ) માં ધોરણઃ —૧ થી ધોરણઃ —૮ સુધીની લધુમતિ સમુદાય પ્રમાણે તાઃ- ૩૧/૦૩/૨૦૨૨ ની સ્થિતિએ સંખ્યા લખવી. આંગણવાડી ન હોય, તો તે ખાનું ખાલી છોડવું. 

નંબર ૪.૪ માટે જો ગુજરાતી માધ્યમવાળી શાળા હોય, તો ધોરણઃ —૧ થી ધોરણઃ —૮ ના બાળકોની સંખ્યા મીડીયમ – ૧ માં લખવી. આંગણવાડી ન હોય, તો તે ખાનું ખાલી છોડવું 

√ અહીં ખાસ નોંધવું કે મુદ્દા નં ૪.૨ ( એ ), મુદ્દા નં ૪.૩ અને મુદ્દા નં ૪.૪ ની સંખ્યા સમાન જ હોવી જોઇએ. 

√ મુદ્દા નંબર ૪.૫ ( એ ) માં તાઃ- ૩૧/૦૩/૨૦૨૨ ની સ્થિતિએ ધોરણઃ —૧ થી ધોરણઃ —૮ માં પુનઃપ્રવેશ આપેલ બાળકો ની જાતિવાર / કુમાર કન્યાવાર સંખ્યા લખવી. આંગણવાડી ન હોય, તો તે ખાનું ખાલી છોડવું. 

√ મુદ્દા નંબર ૪.૫ ( બી ) માં પુનઃનામાંકન દર્શાવેલ ( કોષ્ટક ૪.૫ એ ) બાળકો પૈકી ધોરણઃ -૧ થી ધોરણઃ -૮ ના બાળકોની લઘુમતિ સમુદાયવાર / કુમાર કન્યાવાર સંખ્યા લખવી. આંગણવાડી ન હોય, તો તે ખાનું ખાલી છોડવું. 

√ મુદ્દા નંબર ૪.૬ માં શાળામાં ભણતા દિવ્યાંગ બાળકોની પ્રકાર પ્રમાણે / કુમાર કન્યાવાર તાઃ- ૩૧/૦૩/૨૦૨૨ ની સ્થિતિએ ધોરણઃ —૧ થી ધોરણઃ -૮ ની સંખ્યા લખવી. ( દિવ્યાંગ બાળકોના ખાસ શિક્ષકની મદદ લેવી ) 

વિભાગઃ– ૫ બાળકોને આપવામાં આવેલ પ્રોત્સાહક લાભો અને સવિધા ( ફકત સરકારી શાળા અને ગ્રાન્ટેડ શાળા માટે ) 

•  આ વિભાગ ખાનગી શાળાએ ભરવાનો નથી. 

• મુદ્દા નંબર ૫.૧ માં ધોઃ— ૧ થી ૫ ( પ્રાથમિક વિભાગ ) ના બાળકોને તાઃ– ૩૧/૦૩/૨૦૨૨ ની સ્થિતિએ સરકાર તરફથી મળેલ વિવિધ પ્રોત્સાહક લાભોની જાતિવાર / કુમાર કન્યાવાર સંખ્યા દર્શાવવી. 

• મુદ્દા નંબર ૫.૨ માં ધોઃ- ૬ થી ૮ ( ઉચ્ચ પ્રાથમિક વિભાગ ) ના બાળકોને તાઃ– ૩૧/૦૩/૨૦૨૨ ની સ્થિતિએ સરકાર તરફથી મળેલ વિવિધ પ્રોત્સાહક લાભોની જાતિવાર / કુમાર કન્યાવાર સંખ્યા દર્શાવવી. ધોઃ— ૧ થી ૫ વાળી શાળાએ આ ભાગ ખાલી છોડવો. 

• મુદ્દા નંબ ૨ ૫.૩ માં શાળાના દિવ્યાંગ બાળકોને સરકાર તરફથી આપવામાં આવેલ વિવિધ સામગ્રી / સહાયની આંકડાકીય માહિતી દર્શાવવી.

Ayushman Bharat is an attempt to move from sectoral and segmented approach of health service delivery to a comprehensive need-based health care service. This scheme aims to undertake path breaking interventions to holistically address the healthcare system (covering prevention, promotion and ambulatory care) at the primary, secondary and tertiary level. Ayushman Bharat adopts a continuum of care approach, comprising of two inter-related components, which are:

Health and Wellness Centres (HWCs)

Pradhan Mantri Jan Arogya Yojana (PM-JAY)

WHAT’S COVERAGE UNDER PMJAY AYUSHMAN BHARAT YOJANA?

Thus, PM-JAY has been rolled out for the bottom 40 per cent of poor and vulnerable population. In absolute numbers, this is close to 10.74 crore (100.74 million) households. The inclusion of households is based on the deprivation and occupational criteria of the Socio-Economic Caste Census 2011 (SECC 2011) for rural and urban areas, respectively. This number also includes families that were covered in the RSBY but were not present in the SECC 2011 database.

Rural Beneficiaries:

Out of the total seven deprivation criteria for rural areas, PM-JAY covered all such families who fall into at least one of the following six deprivation criteria (D1 to D5 and D7) and automatic inclusion(Destitute/ living on alms, manual scavenger households, primitive tribal group, legally released bonded labour) criteria:

D1- Only one room with kucha walls and kucha roof

D2- No adult member between ages 16 to 59

D3- Households with no adult male member between ages 16 to 59

D4- Disabled member and no able-bodied adult member

D5- SC/ST households

D7- Landless households deriving a major part of their income from manual casual labour

Urban Beneficiaries:

For urban areas, the following 11 occupational categories of workers are eligible for the scheme:

Street vendor/ Cobbler/hawker / other service provider working on streets

Construction worker/ Plumber/ Mason/ Labour/ Painter/ Welder/ Security guard/ Coolie and other head-load worker

Sweeper/ Sanitation worker/ Mali

Home-based worker/ Artisan/ Handicrafts worker/ Tailor

Transport worker/ Driver/ Conductor/ Helper to drivers and conductors/ Cart puller/ Rickshaw puller

The individuals who claim a two, three or four-wheeler or a mechanized angling pontoon

The individuals who claim motorized cultivating gear

The individuals who have Kisan cards with a credit breaking point of Rs.50000

Those utilized by the legislature

There will be a total of 1350 types of surgeries, screenings and procedures in India. Every poor citizen of the country will get free benefits from major illnesses and major operations as well as hospitalization. The operation included bypass surgery, cataract, corneal grafting, orthoplasty, chest fracture, urological surgery, cesarean delivery, dialysis, spine surgery, brain tumor surgery and various cancer surgeries.

All the government and hospitals in the country connected with Ayushman Bharat Yojana will get benefit of Ayushman Bharat Yojana. The connection of 8,000 hospitals has been completed in Ayushman Bharat Yojana and the Government aims to connect 20,000 hospitals. So that the poorest family living in any corner of the country can access health services near their home. HOW TO APPLY PMJAY AYUSHMAN BHARAT YOJANA?

There is no special Ayushman Bharat registration procedure pertaining to PMJAY. This is because PMJAY applies to all beneficiaries as identified by the SECC 2011 and those who are already part of the RSBY scheme. However, here’s how you can check if you are eligible to be a beneficiary of PMJAY.

1. Visit https://www.pmjay.gov.in/ and click on ‘Am I Eligible’

2. Enter your mobile number and the CAPTCHA code and click on ‘Generate OTP’

3. Then select your state and search by name/ HHD number/ ration card number/ mobile number

4. Based on the search results you can verify if your family is covered under PMJAY

Alternatively, to know if you are eligible for PMJAY you can approach any Empanelled Health Care Provider (EHCP) or dial the Ayushman Bharat Yojana call centre number: 14555 or 1800-111-565

WHICH CRITICAL DISEASES COVERED UNDER PMJAY AYUSHMAN BHARAT YOJANA?

PMJAY helps households access secondary and tertiary care via funding of up to Rs. 5 lakh per family, per year. This assistance is valid for day care procedures and even applies to pre-existing conditions. PMJAY extends coverage for over 1,350 medical packages at empanelled public and private hospitals.

Some of the Critical illnesses that are covered are as follows.

Prostate cancer

Coronary artery bypass grafting

Double valve replacement

Carotid angioplasty with stent

Pulmonary valve replacement

Skull base surgery

Laryngopharyngectomy with gastric pull-up

Anterior spine fixation

Tissue expander for disfigurement following burns

PMJAY has a minimal list of exclusions. They are as follows:

Individual diagnostics (for evaluation)

PM-JAY is completely funded by the Government and costs are shared between Central and State Governments. The Government of India decides a national ceiling amount per family that is used to determine the maximum limit of the central share of the contribution. The actual premium discovered through open tendering process or the maximum ceiling of the estimated premium decided by Government of India for the implementation of PM-JAY, whichever is less, would be shared between Central Government and States/UTs in the ratio as per the extant directives issued by Ministry of Finance, from time to time. In addition, administrative cost for implementing the scheme at the State level is also provided under the scheme and shared between Centre and State in the same sharing pattern.
